Care Manager

Care Manager Job Description

Are you a compassionate caregiver who is dedicated to providing top-notch care for seniors in the Grand Forks area? All Embracing Home Care is seeking a Care Manager to join our team and help us continue to provide exceptional care to our clients.

As a Care Manager, you will be responsible for overseeing the care of our clients and ensuring that their needs are met on a daily basis. You will work closely with our caregivers to create personalized care plans for each client, and you will be responsible for monitoring the progress of these plans and making adjustments as needed. In addition, you will serve as a liaison between our clients, their families, and our caregivers to ensure that everyone is on the same page and that our clients are receiving the best possible care.

The ideal candidate for this position will have at least two years of experience working as a caregiver, as well as strong communication and organizational skills. You must be able to work independently and make decisions quickly, and you must have a passion for helping others and improving the lives of seniors in our community.
