All Embracing Home Care is seeking a compassionate and organized Program Coordinator for Individuals with Developmental Disabilities to oversee and coordinate person-centered services for individuals with developmental disabilities. This role ensures that our individuals receive high-quality, individualized care that promotes independence and integration into the community. The Program Coordinator will work closely with individuals, families, service providers, and regulatory agencies to develop and implement individualized service plans while ensuring compliance with North Dakota state requirements for a Developmental Disabilities Program Manager (DDPM).
Key Responsibilities:
- Service Coordination: Ensure individuals receive appropriate, high-quality care and services that align with their needs, preferences, and goals.
- Person-Centered Planning: Develop, implement, and monitor individualized service plans (ISPs) in collaboration with our individuals, families, and interdisciplinary teams.
- Regulatory Compliance: Ensure all services adhere to North Dakota's state and federal regulations governing developmental disability services.
- Team Leadership: Provide supervision, training, and guidance to Care Managers to promote best practices in service delivery.
- Advocacy & Support: Act as an advocate for individuals, ensuring their rights and choices are respected while fostering self-determination.
- Budget & Resource Management: Effectively manage program resources and budgets to meet the individuals needs and maintain operational efficiency.
- Documentation & Reporting: Maintain accurate and confidential records, including progress notes, assessments, incident reports, and required state documentation.
Minimum Qualifications:
- Education: Bachelor’s degree in social work, psychology, counseling, nursing, occupational therapy, or a related human services field.
- Licensure: Valid driver’s license and the ability to meet agency driving requirements.
Preferred Qualifications:
- Knowledge: Familiarity with North Dakota’s Developmental Disabilities Program Management (DDPM) requirements, Medicaid waiver programs, and person-centered planning.
- Experience: At least one year of direct care experience working with individuals with developmental disabilities or mental health conditions.
- Advanced Education: Master’s degree in a related field.
- Certifications: Qualified Intellectual Disabilities Professional (QIDP) certification or equivalent.
- Additional Experience: Experience in program management, staff supervision, or working within home- and community-based services (HCBS).

Work Environment & Schedule:
- Setting: Office-based with regular visits to individuals’ homes, community sites, and service providers.
- Travel: Some travel required.
- Hours: Full-time position; occasional evenings and weekends may be needed based on individual needs. Along with participation in the on-call weekend rotation.
